Referrers / clinicians

 

We offer New Zealand’s only national residential DBT programme for people usually diagnosed with Borderline Personality Disorder, Post Traumatic Stress Disorder, and related challenges.

District Health Boards (DHB) clinicians and Accident Compensation Corporation (ACC) approved clinicians can refer to our DBT programme. Funding must be in place before referring.

When determining if our DBT service is suitable, we consider:

  1. The ‘application form’ (a form the applicant can complete alone or with the referrer).
  2. The ‘referral form’ (to be completed by the referrer)
  3. A physical health report from the GP including medication/treatments. If a psychiatrist is involved we will need their agreement to continue their involvement.
  4. A summary of the applicant’s history; we can receive this from various health professionals, such as psychiatrists, psychologists, therapists and counsellors. We accept this information in many formats e.g. assessments, reports etc., covering:
    1. Psychosocial history, particularly any trauma history and current symptoms
    2. Current and previous diagnoses
    3. Treatments / interventions received and response / outcomes
    4. Detailed history of suicide attempts and self-harm
    5. Personality assessment, if available
    6. Details of any intellectual impairment if known or suspected and IQ Assessment, if required
